Participants:
Current focus on uninsured and underserved adults with chronic
medical conditions. Regional population is 700,000, with 420,000
residents in Escambia and Santa Rosa counties. CHIN participants
include hospital systems, behavioral health provider, regional
FQHC, faith-based free clinics, dental clinic, county health
departments, and social service agencies.
Status:
Active. CHIN provides electronic linkage and information exchange
for 18 providers in two-county community health safety net
serving poor and uninsured patents. |
