Based in Pensacola, with current coverage
in Escambia & Santa Rosa counties, and plans for future
operation in Okaloosa, Walton, Gulf, Bay, Holmes, and Washington
counties.
Participants:
Current focus on uninsured and underserved adults with chronic
medical conditions. Regional population is 700,000, with 420,000
residents in Escambia and Santa Rosa counties. CHIN participants
include hospital systems, behavioral health provider, regional
FQHC, faith-based free clinics, dental clinic, county health
departments, and social service agencies.
Status:
Currently have daily, automated HIE activity linking 15 providers.
RHIO goal is to operate a secure electronic network that allows
authorized users to view integrated data on patients' health
status and treatment. This information is used to improve
coordination of care, eliminate duplicate treatment and unnecessary
referrals, improve efficiencies and reduce costs to operate
the community health safety net which serves the poor and
uninsured. |
